I hereby grant Rx Corp and my prescribing physician the right to access any medical data from the medical questionnaire submitted, including but not limited to information about mental health condition, HIV/AIDS, alcohol or drug addition, as well as any cases of substance abuse. I authorize my treating physician to disclose any medical information deemed necessary by my prescribing physician for that person to make the decision about the request submitted.
